Fast Flicks Hockey League welcomes you to the PA Document Repository Service with National Center for Safety Initiatives (NCSI). Please follow the directions below to start the process.

Simply go to: Sports Engine Document Storage

Step 1: Scroll to bottom of page and create new account

Step 2: Upload your three documents

Step 3: Provide Legal Authorization and Certification

It is important when entering your name, that you use your full legal name as written on your current ID (state driver’s license, passport or state ID with name and DOB). Please also verify that your social security number was typed correctly. Using nicknames or failing to type a correct social security number may delay or invalidate the upload of your documents.

Three documents that need to be obtained and then uploaded to be in compliance with PA Act 153 (do either number 3 or number 4, depending on your length of residency in Pennsylvania): 

1. Child Abuse Clearance:https://www.compass.state.pa.us/cwis/public/home

2. State of PA Clearance https://www.psp.pa.gov/pages/criminal-history-background-check.aspx

3. FBI Report if in PA for less than 10 years of residency: http://www.dhs.pa.gov/citizens/clearancesbackgroundchecks/

4. Affidavit that states you have been in PA for more than 10 years of residency  Fast Flicks Affidavit Waiver
